Rise Above Foundation provides supportive and transitional housing services designed to help individuals and families move from housing instability toward long-term independence. Our programs focus on safety, structure, accountability, and practical support while empowering participants to build sustainable housing and life stability.

Supportive & Transitional Housing Assistance

We assist participants in accessing safe and stable housing environments while they work toward independent living. Supportive and transitional housing assistance provides a structured setting that allows individuals and families to focus on stabilization, personal growth, and future planning.

Participants receive guidance and support as they transition from unstable or temporary housing into more permanent living arrangements, with an emphasis on responsibility, safety, and readiness for independence.

Housing Stabilization & Retention Support

Maintaining housing is essential to long-term stability. Rise Above Foundation supports participants in developing the skills and habits necessary to retain housing over time.

Housing stabilization and retention support may include:

  • Support with understanding lease expectations and tenant responsibilities

  • Assistance in building routines that support housing stability

  • Problem-solving support to address housing-related challenges

  • Encouragement of proactive communication and planning

Our goal is to reduce housing disruptions and support participants in sustaining stable housing long term.

Life Skills Development & Independent Living Preparation

Life skills development is a core component of our services. Participants are supported in strengthening the skills necessary for independent living and self-sufficiency.

Areas of focus may include:

  • Daily living and household management skills

  • Time management and organization

  • Financial awareness and budgeting basics

  • Healthy routines and decision-making

These skills help participants build confidence, independence, and readiness for long-term housing success.

Individualized Goal Planning & Accountability Support

Each participant works toward individualized goals based on their unique strengths, needs, and circumstances. Goal planning is collaborative, structured, and focused on measurable progress.

Accountability support includes:

  • Setting clear, achievable goals

  • Tracking progress and milestones

  • Regular check-ins to support follow-through

  • Encouragement of personal responsibility and growth

This approach helps participants remain engaged, motivated, and focused on long-term success.

Advocacy & Coordination with Community Providers

Rise Above Foundation works closely with community providers to ensure participants have access to additional resources and services as needed. Through advocacy and coordination, we help participants navigate systems and connect with appropriate support.

This may include coordination with:

  • Behavioral health providers

  • Substance use treatment programs

  • Medical and primary healthcare providers

  • Supportive housing and employment resources

Services provided by Rise Above Foundation are non-clinical. Clinical, medical, and treatment services are delivered by licensed third-party providers through referral and coordinated support.
